Our office is located in Suite 300. This is in the Lakeview Medical Dental Building on the corner of Union Bay Place NE and NE 45th Place.

To access our suite (300) on the far west end of the building, on the third floor (see image):

• Stairwell access on the far west end of the building – takes you directly to our suite
  • Accessible from the main front parking lot on the 1st floor
• Elevator access on the far east end of the building (in the courtyard)
  • Accessible from the 1st, 2nd and 3rd floors by the main building courtyard
  • Can be accessed from the main front parking lot and the Union Parking Garage parking lot
  • On the third floor, walk around the courtyard, through the doors to the hallway, and down the west corridor to get to our suite
  • Please note, suites are not numbered in order!

Parking is free for all patients and is available in the following locations:

• Upper Burke + Union Parking Garage – accessible off of NE 45th Place (in the back of the building)
• Lakeview Medical Dental Parking lot – accessible off of NE 45th Place (directly in front of building)

Please note that parking in the parking garage behind the building is restricted outside the following hours. You will NOT have access to your car if you park in the lot before or after these times:

• Monday – Friday: 7:00a – 7:00p
• Saturday – 8:00a – 2:00p

Handicap parking is available in both parking lots with ramps and elevators accessible as well.

From I-5:

1. Take Exit 169 toward NE 45th St
2. Head East on NE 45th St (Left turn off of the free way from I-5 N; Right turn off the freeway from I-5 S)
3. Continue going East on NE 45th St for 1 mile
4. At the University Village, turn left to stay on NE 4th St
5. In 0.3 miles, turn left onto NE 45th Pl.
6. Parking can be accessed by 2 driveways immediately to the left after turning onto NE 45th Pl.

From WA-520 W:

1. Take the Montlake Blvd E exit and merge onto Montlake Blvd E
2. Follow Montlake Blvd E for 1.2 miles
3. Montlake Blvd East will merge with NE 45th St – Continue on NE 45th St for 0.3 miles
4. Turn left onto NE 45th Pl.Parking can be accessed by 2 driveways immediately to the left after turning onto NE 45th Pl.
